Eligibility
Inclusion criteria
Inclusion criteria: Patients more than 18 years of age, undergoing elective surgery under general anaesthesia with endotracheal tube, in JIPMER-Puducherry.
Exclusion criteria
Exclusion criteria: 1.Patients requiring rapid-sequence induction. 2.Patients with naso-gastric tube in-situ. 3. Patients with neck swelling, deformities of neck preventing upper oesophageal ultrasound. 4. Patients with oesophageal/gastric pathology/GERD. 5.Patients with air in gastric antrum prior to FMV
Design outcomes
Primary
| Measure | Time frame |
|---|---|
| To compare the incidence of â??comet-tailâ?? artefact with real-time cervical oesophageal ultrasonography and gastric antral ultrasonography. Timepoint: â??Comet-tailâ?? artefacts, a marker of air entry intro the oesophagus will be looked for during face mask ventilation for 3 minutes during induction of general anaesthesia . This will be compared with the â??comet-tailâ?? artefacts in the gastric antrum after the placement of endotracheal tube.Since air has to traverse through oesophagus before entering stomach, a better alternative and more sensitive method would be to look for oesophageal insufflation during face mask ventilation. | — |
Secondary
| Measure | Time frame |
|---|---|
| To identify the lowest peak airway pressure at which â??comet-tailâ?? artefacts appear in the oesophagus during facemask ventilation.Timepoint: This will be looked for in 3 minutes of face mask ventilation during induction.Ability to do face-mask ventilation will be tested using pressure-controlled mode of ventilation with an inspiratory pressure of 10cmH2O, the inspiratory pressure will be stepped up or down in increments of 2 cmH2O to achieve a target tidal volume of 8 ml.kg-1.Lowest peak airway pressure at which comet tail artefacts appear will be noted. | — |
