Eligibility

Inclusion criteria

Inclusion criteria: 1. Patients undergoing surgery for fibrostenosing Crohnâ??s disease 2. Inception patients (new diagnosis of IBD) 3. Patients who have not had any biologic IBD drugs 4. Patients on anti-TNFα therapy (pre and post therapy) 5. Patients who have discontinued biological therapy 6. Patients on ustekinumab (pre and post-therapy) 7. Patients on vedolizumab (pre and post-therapy) 8. Patients who have had a resection in the past 9. Patients with a diagnosis of irritable bowel syndrome

Exclusion criteria

Exclusion criteria: 1. Less than 18 years 2. More than 80 years 3. Unable to consent.

Design outcomes

Primary

MeasureTime frame
Molecular and biomarker studies (multiomics) of surgically resected bowel to better understand the mechanism of fibrosis in Crohnâ??s disease.Timepoint: 1. End of every year. 2. Each patient will be monitored for 4 years.

Secondary

MeasureTime frame
1. Study cytokines, chemokines, transcription factors and biomarkers in fibrotic segments of the resected bowel. 2. Study cell-subsets in blood and resected bowel -help stratify patients at risk of fibrosis. 3. Study for potential targets of therapy to prevent or reverse fibrosis. 4. To compare findings among various cohorts. 5. To assess for any objective evidence of reversal of fibrosis with recession of biomarkers in patients who respond well to biologics. Timepoint: Over a period of 4 years
