Inclusion criteria: Adult patients who have been hospitalized to the inpatient wards in the last 12 months

Exclusion criteria

Exclusion criteria: Children less than 18 years old, Pregnant women, Patients unable to consent due to altered mental status, Patients currently in the Intensive Care Unit
