Inclusion criteria
Inclusion criteria: Persons who are attending the Cardiology and Cardiothoracic surgery units, in the age group of 21 years and above, on oral anticoagulation therapy for minimum one year duration with minimum 5 INR readings in the last one year, can speak and understand English or Kannada
Exclusion criteria
Exclusion criteria: Persons who are on oral anticoagulation therapy for less than 1 year, maintaining TTR >= 60%, cognitively impaired, decline to participate, with terminal illness and pregnant mothers.

| Improvement in knowledge scores which may contribute to better adherence behavior towards oral anticoagulation therapy. Decrease in complication rates in terms of bleeding and thromboembolic complications. Increase in quality of life scores of patients who are on oral anticoagulation therapy as they are cardiovascular diseases. Development of Nurse led anticoagulation clinic with trained staffs to continue the services in the clinic. Timepoint: 36 months | — |
