Eligibility

Inclusion criteria

Inclusion criteria: 1. All adult patient (age more than or equal to 18 years) presenting with respiratory failure requiring invasive mechanical ventilation. 2. Patients able to spontaneously trigger the ventilator

Exclusion criteria

Exclusion criteria: 1. Unable to trigger ventilator 2. Moderate or severe acute respiratory distress syndrome (ARDS) 3. Presence of three or more organ system failures 4. Neurological or neuromuscular disease(CVA,GBS,Mysthaeniaetc) 5. Known Phrenic nerve palsy 6. Patient who cannot be ventilated without neuromuscular blockers 7. Patient in shock requiring 2 or more inotropes 8. Any contraindication to the insertion of a nasogastric tube (for example: recent upper gastrointestinal surgery, esophageal varices). 9. Patients on mechanical ventilation for more than 3 days. 10. Patient not willing to participate

MeasureTime frame
To compare the incidence of diaphragmatic dysfunction (defined as diaphragm thickness fraction less than 20%) in patient ventilated with NAVA as compared to conventional mechanical ventilation.Timepoint: Day 5 or Day of Extubation whichever is earlier

Secondary

MeasureTime frame
1. Change in thickness of Diaphragm 2. Diaphragmatic excursion 3. Diaphragmatic efficiency (NVE and NME) Timepoint: Day 5 or Day of Extubation which ever is earlier
