Eligibility

Inclusion criteria

Inclusion criteria: After percutaneous catheter drainage CT will be repeated on day 3 ,if the residual collection / necrosum is more than 50% of initial value then patients are randomized in to either streptokinase group or hydrogen peroxide group

Exclusion criteria

Exclusion criteria: 1.Indication for emergency laparotomy because of suspected abdominal compartment syndrome, bowel ischemia, bleeding or perforation of a visceral organ 2. Previous drainage or surgical necrosectomy for infected pancreatic necrosis (ERCP with or without papillotomy is allowed). 3. Patients who are allergic to streptokinase. 4. Patients with deranged coagulation profile. 5. Patients with recent history of cerebrovascular accident ( surgery, uncontrolled hypertension, intracranial neoplasm.

Design outcomes

Primary

MeasureTime frame
1.improvement in clinical condition 2.Increased intake orally /NJ/NG 3. Resolution of pancreatic and or peri pancreatic necrosis. Timepoint: 4th day

1.Length of intensive care unit (ICU) and hospital stay 2. Proportion of patients requiring surgical necrosectomy in 2 groups 3. Number and size of catheters required in 2 groups 4. Number of interventions required in 2 groups 5. Catheter-related complications in 2 groups. Timepoint: duration of hospital stay
