Eligibility

Inclusion criteria

Inclusion criteria: 1) Adult patients ( >18 year old) 2) Patients receiving controlled mechanical ventilation while receiving sedation ± paralysis 3) Poor acoustic window for apical four chamber or parasternal short axis or both views as judged by treating doctor during point of care ultrasound/ echocardiography examination.

Exclusion criteria

Exclusion criteria: 1) Patients triggering breath while on ventilator. 2) Patients on pressure support mode (or any other spontaneous mode) of ventilation 3) Prone position ventilation 4) Patients getting >6 ml kg tidal volume. 5) Expiratory flow limitation as judged by incomplete expiration on flow time scalar.

1. Change in Carotid Doppler peak velocity (CDPV) with 8 second end expiratory hold. 2. Change in IVC diameter with 8 second end expiratory hold 3. Auto-PEEP in cm H2O as measured on ventilator after 8 seconds of expiratory hold. Timepoint: At 8 seconds end-expiratory hold;Predefined subgroup: Effect of end-exploratory hold will be compared in following pre-defined subgroups 1. Cyclic interference in acoustic window during tidal ventilation, defined as â?¥ 1 point change in Worst image quality during tidal ventilation. Timepoint: At 8 seconds end-expiratory hold
