Eligibility

Inclusion criteria

Inclusion criteria: a) Patients with selected NCDs (cardiovascular diseases, diabetes mellitus, chronic obstructive pulmonary disease) reporting to the outpatient department of selected health facilities b) Patients who are likely to stay in the study area for at least six months

Exclusion criteria

Exclusion criteria: a) Patients with other NCDs like carcinomas, psychiatric disorders, asthma b) Those who are likely to leave the study area within next six months c) Those who refuse to participate in the study
