Eligibility

Inclusion criteria

Inclusion criteria: patients with perforation peritonitis as cases and patients undergoing elective laparotomy as controls

Exclusion criteria

Exclusion criteria: 1. Patients or relative’s refusal to consent for study 2. History of ICU stay in last 6 months 3. Pre-existing significant cardiac, renal and hepatic diseases 4. Intraoperative massive blood loss or massive transfusion 5. History of CAD, CVA, malignancy, diabetes, hypertension, metabolic syndrome, PVD.

MeasureTime frame
To assess if pre-op and post op brachial artery reactivity (Flow mediated dilation and Hyperemic velocity) can predict in-hospital mortality in perforation peritonitis patients undergoing emergency laparotomy.Timepoint: brachial artery reactivity measured preoperatively, immediated postoperatively, 24 and 48hrs postoperatively.

Hospital length of stay b) acute kidney injury and requirement of renal replacement therapy, c) vasopressor requirement (number of days on vasopressor) d) mechanical ventilation requirement (number of days on mechanical ventilation) e) And to assess whether change in brachial artery reactivity correlates with lactate clearance in perforation peritonitis patients undergoing emergency laparotomy.Timepoint: patient will be followed up daily till 28 days or discharge or death whichever is earlier to record the above parametres
