Eligibility
Inclusion criteria
Inclusion criteria: 1.Age >= 55 years and 1 or more of the following advanced chronic illnesses: Chronic obstructive pulmonary disease (2 of the following: baseline arterial partial pressure of carbon dioxide > 45 mm Hg, cor pulmonale, episode of respiratory failure within the preceding year) Congestive heart failure (New York Heart Association Class IV symptoms and left ventricular ejection fraction Cirrhosis (confirmed by imaging studies or documentation of esophageal varices) and 1 of the following: hepatic coma, Child class C liver disease, Child class B liver disease with gastrointestinal bleeding Cancer (metastatic cancer or stage IV lymphoma) OR 2.Any patient >= 80 years of age admitted to hospital from the community because of an acute medical condition. OR 3.You answer â??noâ?? to the following question: â??Would I be surprised if this patient died within the next year?â??
Exclusion criteria
Exclusion criteria: 1.Patients or family not competent [Competence will be judged by the ability of the patient to understand and discuss illness, current treatment and possible future treatment] 2.Patients expected to die or be discharged within 24 hours. 3.Patients with no family/ caregiver.
