Eligibility

Inclusion criteria

Inclusion criteria: American Society of Anesthesiologists (ASA) physical status I and II and scheduled to undergo surgery in supine position with neutral head position, under general anaesthesia with controlled hypotension will be enrolled for this randomised non-inferiority trial.

Exclusion criteria

Exclusion criteria: Preexisting cerebral pathology, Cognitive disability, Carotid artery stenosis, Cardiac disease, Orthostatic hypotension, Sensitivity to study drugs.

Primary

MeasureTime frame
To compare the occurrence of cerebral desaturation events caused by use of dexmedetomidine and nitroglycerin for controlled hypotensive anaesthesia.Timepoint: 30 minutes

Secondary

MeasureTime frame
To compare the effects of dexmedetomidine and nitroglycerin on left and right regional cerebral oxygen saturation, haemodynamics (blood pressure and heart rate) and postoperative cognitive decline, when used for controlled hypotensive anaesthesia.Timepoint: MMSE score will be re-evaluated after 3 days to look for postoperative cognitive decline (POCD). Any decrease in score 2 points will be defined as POCD.
