Inclusion criteria

Inclusion criteria: Type 2 Diabetic patients above 18 years of age admitted in the in Department of Medicine at Kasturba Hospital, Manipal and DM patients glycosylated Hemoglobin should be more than 7%.

Exclusion criteria

Exclusion criteria: Type 1 Diabetic patients, Gestational diabetes, Bedridden patients and Patients who are not willing to participate in the study.

Improvement in Quality of life QOL, knowledge and laboratory values expected.Timepoint: 6 months

Betterment in the Patient health after the Counselling and the Medication order review.Timepoint: At the End of followup of each patient.
