Inclusion criteria
Inclusion criteria: 1. Age>= 18 years 2. Patient with a diagnosis of heart failure (NYHA Class I II, III IV) and may having co-morbidities like HTN, Cardiomyopathies, Valvular heart disease and taking medicines for HF for at least 1 months. 3. Ongoing assessment for revascularization, cardiac resynchronization and heart transplantation. 4. Patients who are willing to participate. 5. Patients who can read and write Hindi or English. 6. Patient who have their personal/ landline number. 7. Patients who have smart phone with android operating system
Exclusion criteria
Exclusion criteria: 1. End-stage heart disease, chronic renal failure, dementia and other psychiatric illnesses. 2. Patient having dyspnea other than cardiac cause (COPD, Asthma, Chronic bronchitis, anemia) 3. Illnesses that are likely to lead to hospitalization & death in near future.
