Health Condition 1: null- The patients of heart failure with NYHA class I to class IV requiring frequent monitoring at OPDs

Interventions

Intervention1: All the patients meeting inclusion criteria will be taken as subjects for interventions. No control will be there: Structured teaching program regarding self management of heart failure

Eligibility

Inclusion criteria

Inclusion criteria: all patients of heart failure with NYHA class I to ambulatory class IV with ejection fraction less than 40% at the time of enrolment in the study

Exclusion criteria

Exclusion criteria: Patients of unsound mind and have unstable heart failure
