Inclusion criteria
Inclusion criteria: 1. All ex-servicemen and their families who attended the clinic over the age of 60 years 2. All ex-servicemen and their families who are on 5 or more regular drugs.
Exclusion criteria
Exclusion criteria: 1. Ex-servicemen and their family members who are below the age of 60. 2. Ex-servicemen and their families who are unable to give clear account of themselves, unable to read or understand the questionnaire with the assumption that they will not be able to use the tool kit effectively.
