Inclusion criteria

Inclusion criteria: 1.Patient who has given written informed consent. 2.Patients with diagnosed CKD (As per treating physician and/orAlbuminuria - AER >30 mg/24 h and/or Electrolyte and other abnormalities), attending nephrology OPD

Exclusion criteria

Exclusion criteria: 1. CKD patients admitted in ward 2. Patients who are planned for renal transplant
