Inclusion criteria

Inclusion criteria: 1.New Patients of age group above 18 years who are coming for medicine outpatient department services at government and private secondary level health care centers and are willing to provide written informed consent are included. 2.New Patients of age group 2-18 years who are coming for paediatric outpatient department services are included and informed consent is obtained from their parents or guardians

Exclusion criteria

Exclusion criteria: 1.Patients who are coming for follow-up. 2.In patients brought to medicine and paediatric outpatient patient department.
