Eligibility

Inclusion criteria

Inclusion criteria: 1 Adults with diabetes registered and regularly attending (at least once a month) the NCD clinics of the study setting. 2 All Junior Public Health Nurses in charge of the subcentres under study.

Exclusion criteria

Exclusion criteria: Patients 1.Not willing to give consent for participation. 2.Presence of other comorbidities like malignancy, renal or hepatic disease. 3.Pregnant women. 4.Inability to perform physical activity like bedridden patients, history of a cardiac event in past 12 months. 5.Cognitive impairment, dementia or mental health problems.
