Eligibility
Inclusion criteria
Inclusion criteria: a) Patients above the age of 60 years with at least one of the following Non-communicable diseases; diabetes, hypertensive, Coronary artery disease b) Patients who give their consent
Exclusion criteria
Exclusion criteria: a) Patients who are critically ill, not likely to survive through the study duration. b) Patients who are living far away and are not likely to come for follow-up visits c) Patients who are totally dependent on a care-giver. (e.g., bed-ridden patients, poor cognition) d) Patients who do not have sufficient education to follow the study protocols
