Inclusion criteria
Inclusion criteria: 1. Adult ambulant outpatients of both genders diagnosed with chronic heart failure, based on standard clinical criteria and clinical judgement of the treating physician, with stable clinical signs and symptoms of heart failure for at least 3 months 2. Presence or absence of diabetes will be based on whether or not the patient has been diagnosed with diabetes prior to study entry. If diabetes is present, patient will be included only if he has had a history of diabetes for at least 6 months 3. A written, signed and dated data sharing consent from all participants before start of survey procedures
Exclusion criteria
Exclusion criteria: 1. Hospitalized patients of heart failure 2. Heart failure patients of the following etiologies - a. congenital heart disease b. valvular heart disease c. cor-pulmonale, including primary pulmonary hypertension
