Inclusion criteria

Inclusion criteria: 1.Adult patients with Type 2 Diabetes mellitus, aged 18 years or more 2.Resident of the PHC service area and receiving treatment from chronic disease clinic at the PHC

self-care behavior will be measured as score (calculated as the sum of days of items 1 to 10 divided by 10, on SDSCA scale)Timepoint: 6months

MeasureTime frame
Stages of readiness for change (in diet, exercise and foot care domains) will be measured as % of subjects in preaction and action/ maintenance phase.BP, BMI, age, duration of disease.Association of self-care practices in different domains will be tested with various demographic, socioeconomic, and diabetes related independent variables Timepoint: 6 months
