Inclusion criteria

Inclusion criteria: The patient criteria for inclusion of two cohorts in the retrospective study are:- (a) patients with index admission as Acute Myocardial Infarction, and (b) patients with index admission of Stroke. (c) All age group and gender would be included

Exclusion criteria

Exclusion criteria: No exclusions within the two cohorts

MeasureTime frame
The primary outcome of the project would be a risk score for predicting the readmissions in discharge cases of (a) Acute Myocardial Infarction (b) Stroke. Timepoint: A risk score for predicting the readmissions in discharge cases of (a) Acute Myocardial Infarction (b) Stroke at 30 days

Secondary

MeasureTime frame
The secondary outcome of the project would be a risk score for predicting the readmissions in discharge cases of (a) Acute Myocardial Infarction (b) Stroke. Additional outcome measure could be risk of disability following Stroke.Timepoint: A risk score for predicting the readmissions in discharge cases of (a) Acute Myocardial Infarction (b) Stroke at 90 days & 1 year
