Health Condition 1: null- suffering from acute pancreatitis predicted to be severe in form

Interventions

Intervention1: Thoracic epidural: Standard recommended treatment of pancreatitis while using thoracic epidural block for providing analgesia. The technique for providing segmental thoracic epidural b
while using parenteral opiod and paracetamol for providing analgesia.

Eligibility

Inclusion criteria

Inclusion criteria: Patients once diagnosed as acute pancreatitis by the surgeon will be enrolled in the study. Those with features predicting severe acute pancreatitis will be included in the study. The predictors for severe attack will include presence of SIRS on admission or 48 hours later; or presence of obesity; or clinical suspicion of severity; or Glasgow (Imrie) score or Ransonâ??s score of 3 or more, or Acute Physiology And Chronic Health Evaluation (APACHE) II score greater than 8 within 48 hours after admission. All of these indicators have been previously established and are recommended to predict a severe attack.

Exclusion criteria

Exclusion criteria: Failure of epidural block or any contraindication to performance of epidural block.

Primary

MeasureTime frame
Aggregate SOFA: calculated by summing the worst scores for each of the organ system during ICU stay.Timepoint: Aggregate SOFA: calculated by summing the worst scores for each of the organ system during ICU stay.

Secondary

MeasureTime frame
duration of hospitalization, duration of ICU stay, need of mechanical ventilation and occurrence of in-hospital mortality, if any, will also be recorded.Timepoint: from admission to discharge from hospital as well as ICU.
