Inclusion criteria: 1 Hospital staffs who are involved in HIS 2 Two Nurses from each specialty wards 3 Doctors [interns]writing patients discharge summaries 4 Clerks of medical records department billing section and laboratory

Exclusion criteria

Exclusion criteria: 1 Hospital staffs who are not involved in HIS 2 Hospital staffs from Radiology department and Administrative offices.
