Inclusion criteria

Inclusion criteria: All patients undergoing either elective or emergency surgeries in major operation theatres.

Exclusion criteria

Exclusion criteria: Procedures conducted solely under local anaesthesia will be excluded

To see the rate of occurrence of sentinel event indicators and rate based indicators. To monitor the quality and appropriateness of patient care Identify areas of deficiency or risks Timepoint: During the conduct of anaesthesia, postoperatively in the recovery room/ICU and 48hrs postoperatively.
