Health Condition 1: null- The study will enroll patients with pancreatic head and periampullary disease, in whom a pancreatic head resection is anticipated

Intervention1: pylorus resecting pancreaticoduodenctomy: Those randomised to this arm will undergo only a pylorus ring excision as against a distal gastrectomy in classical whipples

Eligibility
Inclusion criteria
Inclusion criteria: • Patients with a diagnosis of o Resectable periampullary carcinoma, duodenal tumor and carcinoma head of pancreas o Chronic pancreatitis with suspicious mass in head of pancreas o Patients with borderline resectability will also be included in both arms. • Patient requiring a pancreaticoduodenectomy • ASA Grade I-II patients
Exclusion criteria
Exclusion criteria: • Metastatic or unresectable disease • Patient unwilling to consent for the study • Patients who necessarily will need gastric resection due to infiltration of stomach etc. • Patients with severe comorbidity such as myocardial infarction, respiratory disorder that required oxygen inhalation, liver cirrhosis, hemodialysis (ASA grade III or more) • Concomitant other malignancy
