Inclusion criteria
Inclusion criteria: Already diagnosed patients with type 2 diabetes mellitus
Exclusion criteria
Exclusion criteria: Terminal or debilitating illness; advanced cancers or disabling stroke Those with dementia and other psychiatric illness â?? unwilling to cooperate Non consenting patients

| 1.To determine feasibility of using CHWs to deliver supervised structured interventions to patients with type 2 diabetes mellitus. a.No of patient contacted during each visit. b.Ability to carry out measurements( weight, BP, FBS, PPBS) c.Proportion of patients with actionable values for whom physician opinion was taken by a phone call. 2.To achieve optimal glycemic control (reduction in HbA1c) in the intervention group and standard care group.
