Inclusion criteria

Inclusion criteria: Adult males and females aged between 25 and 85 years and above with a diagnosis of "first-ever-stroke". WHO definition of stroke will be accepted as the inclusion criteria- Stroke(CVD) event is described as rapidly developing signs of focal disturbance of cerebral function lasting more than twenty four hours (unless interrupted by surgery or death) and of presumed vascular origin. The definition includes patients presenting with clinical signs and symptoms of subarachnoid hemorrhage, intracerebral hemorrhage, thrombosis and embolism.

Exclusion criteria

Exclusion criteria: Past history of stroke, recurrent stroke, TIA will be excluded.

MeasureTime frame
: Stroke patient?s Quality of life after stroke will be measured by Barthel Index and Modified Rankin Scale at 28 days, 6 months and 1 year post stroke. Caregiver burden will be assessed by the Bakas Caregiver Outcome scale which has a set of 15 questionsTimepoint: 28 days, 6 months and 1 year post stroke.;Morbidity status at 28days, 6 months and 1 year by Modified Rankin Scale and Barthel Index will be measured. Caregivers stress at 28 days, 6 months and 1 year by Bakas Caregiver Outcome Scale and Caregiver Stress Index will be measured.Timepoint: 28 days, 6 months and 1 year post stroke
