Brief summary

Hypertension remains a major risk factor for heart disease which is amendable to lifestyle medicine through optimisation of nutrition, exercise, sleep, stress, social connection and minimising harmful substances. Digital health programs are growing in popularity as useful tools to facilitate healthy behavioural changes, but formal evaluations of their therapeutic benefits are scarce. This pilot study aims to assess how this digital health anti-hypertension program (Lyfy) facilitates healthy diet and physical activity in patients with high blood pressure in Australian general practice settings. Our hypothesis is that implementing this digital health program is both feasible and acceptable; It will also improve the blood pressure and healthy behavioural changes of the participants.

The Lyfy hypertension management platform is an AI-enabled digital health intervention designed to support the management of blood pressure through systematic analysis of personal health data, generation of individualised care plans, and engagement with healthcare professionals. The platform interprets user-entered blood pressure measurement and other health metrics to identify relevant trends, produces tailored evidence-based lifestyle prescriptions (DASH-based diet & exercise prescription), and provides general practitioners with summarised progress data to inform clinical decision-making with patient’s consent. Its model incorporates multi-disciplinary input from accredited dieticians, physiotherapists, exercise physiologists, and lifestyle medicine trained general practitioners, and adjust recommendations as user data evolves. All participants will have access to the intervention for 12 weeks and their engagement will be measured by Lyfy backstage in terms of frequency of login and completion rate of the lifestyle prescription. They will interact with the Lyfy platform via a smartphone app for 10 - 15 minutes a day on average to receive their lifestyle prescriptions including: - Exercise prescription in FITT format: Frequency, Intensity, Type, Time (duration) with short demonstration videos for certain exercise types. The exercise type ranged from cardio such as brisk walking/jogging/cycling to resistance training of chest, back, upper & lower limbs as well as isometric exercise such as plank & wall squat. - DASH (Dietary Approach to Stop Hypertension) diet-based meal recommendation as an interactive interface for visualisation and calculation of macronutrients. - Sleep hygiene tips sent as short messages via the Lyfy app. They will also receive notifications via the app to monitor their blood pressure on regular basis and log the measurement to the app. The Lyfy algorithm will provide them with clinical insight based on the individual blood pressure readings as well as their overall trend. The clinicians will be given access to the Lyfy clinician portal as a web page. They will be able to monitor the progress of their participating patients and check the completion rate of suggested lifestyle modifications. They also have the option to provide asynchronous feedback and adjust the lifestyle prescription accordingly. The average time spent by the clinicians on each participating patients would be approximately 5 minutes per fortnight. Each clinician would have an average of 10 participants. They can also log into the portal to visualise patients' input of blood pressure readings during the consultation to provide better informed clinical decisions.

Inclusion criteria

Participants will be eligible for inclusion if they meet the following criteria: • Adults aged greater or equal to 18 years old, including both males and females. • Elevated blood pressure within the target range, defined as a systolic blood pressure of 130–160 mmHg and/or a diastolic blood pressure of 80–100 mmHg, based on the mean of three seated measurements taken after a 5 minute rest period, either in the clinic or at home

Exclusion criteria

Participants will be excluded if they meet any of the following conditions: • Secondary hypertension or elevated blood pressure attributable to a cause other than essential hypertension. • Poorly controlled diabetes, defined as HbA1c > 8%. • Recent cardiac event within the past 6 months (e.g., myocardial infarction, unstable angina, or heart failure exacerbation).
