Brief summary

This trial evaluates the feasibility and preliminary effectiveness of delivering iCBT for insomnia prior to Reboot Online. It will assess whether targeting sleep first leads to greater improvements in pain, psychological wellbeing, and functioning compared with Reboot alone. We hypothesis that iCBT insomnia + Reboot Online will be a feasible model of care for participants reporting chronic pain and sleep disturbance and will result in significant reductions in pain interference and pain disability.

Interventions

iCBT Insomnia is an online cognitive behavioural therapy (CBT) program aimed to improve sleep quality and efficiency. It is a self paced 4 lesson course (approximately 1 hour each) delivered over 4 weeks targeting insomnia in adults. Lesson content is presented via an illustrated comic-style story of a fictional male character with insomnia who learns about his insomnia, how it is maintained, and how it can be managed using cognitive and behavioural therapy skills Reboot online is a multidiscip

iCBT Insomnia is an online cognitive behavioural therapy (CBT) program aimed to improve sleep quality and efficiency. It is a self paced 4 lesson course (approximately 1 hour each) delivered over 4 weeks targeting insomnia in adults. Lesson content is presented via an illustrated comic-style story of a fictional male character with insomnia who learns about his insomnia, how it is maintained, and how it can be managed using cognitive and behavioural therapy skills Reboot online is a multidisciplinary online pain management program aimed to improve self-management of pain using non pharmaceutical strategies. It is a self paced 8 lesson (approximately 1 hour each) program delivered over 16 weeks. Lesson content is presented via an illustrated comic-style story of a fictional female character with chronic pain who learns about her chronic pain and pain management strategies that combines iCBT, physiotherapy, multidisciplinary education (medical, nursing, occupational therapy, dietetics), and guided relaxation and movement. Reboot onlien will be offered after iCBT. All interventions and data collection will be delivered online via the ‘THIS WAY UP’ online treatment clinic platform. Both the iCBT Insomnia and Reboot online courses will be administered via the ‘THIS WAY UP’ website (www.thiswayup.org.au). Data regarding program access, adherence and completion is collected via the THISWAYUP platform. Both courses are freely available and include clinician (allied health clinician specialising in pain managment or mental health) email/phone support on request.

Inclusion criteria

• A diagnosis of chronic pain (pain > 3months) with self-report of sleep disturbance and confirmed during telephone screening • Insomnia Severity Index (ISI) greater than or equal to 11 (19) • Internet access and English literacy. • Willingness to provide personal and general practitioner contact details and Consent for research clinician to contact GP if deemed clinically necessary.

Exclusion criteria

High suicide risk or psychosis (frequent suicidal ideation: 2 or 3 on the PHQ-9 item 9 (20) or as determined by a structured interview) • Severe opioid dependence. • Concurrent participation in another pain program. • iCBT completion within past 3 months. • engagement in shift work, or other commitments that interfere with regular night-time sleep patterns or if bedtime is outside the hours of 8pm to 2am more than twice a week • presence of untreated sleep disorder other than insomnia.
