Using routinely collected theatre management system data from a tertiary hospital, patients are classified into three groups based on anaesthetic pathway: block before surgery, theatre block, and no block. The primary outcome is anaesthetic-controlled time, with secondary outcomes including time to readiness for surgery, post-anaesthesia care unit duration, and theatre turnover time. The study is a retrospective observational cohort analysis of cases performed between January 2023 and November 2025. Inclusion criteria

Patients undergoing orthopaedic surgery at Royal Perth Hospital between January 2023 and November 2025. Single orthopaedic procedure recorded per surgical episode based on the International Classification of Diseases, 10th Revision, Australian Modification (ICD-10-AM) coding. Procedures restricted to a predefined list where perioperative analgesia can reasonably be provided using regional anaesthesia. Complete and valid theatre timestamp data available for outcome derivation.

Exclusion criteria

Cases with multiple orthopaedic procedure codes recorded for the same surgical episode. Cases not meeting predefined orthopaedic procedure inclusion criteria. Missing, implausible, or internally inconsistent theatre timestamp data. Repeat surgical episodes for the same patient, with only the first eligible case included in the analysis.
