Brief summary

Breast cancer is one of the leading causes of cancer-related morbidity and mortality among women worldwide and has emerged as the most common cancer among females in Nepal. Despite the availability of screening methods such as mammography, clinical breast examination, and breast self-examination, utilization of breast cancer screening services remains low, particularly due to inadequate knowledge, negative attitudes, sociocultural barriers, and limited intention to seek screening. Early detection through regular screening is crucial for improving treatment outcomes and reducing mortality; however, preventive efforts in Nepal are largely constrained by poor awareness and limited intervention-based evidence.This study aims to assess the effect of a structured health education intervention on the intention to utilize breast cancer screening services among adult women of Pokhara Metropolitan City, Kaski, Nepal. An interventional, quantitative study design will be employed. Two wards of Pokhara Metropolitan City will be randomly selected and allocated into intervention and control groups. A total of 134 adult women aged 40–75 years (67 in each group) will be recruited. The intervention group will receive a health education package comprising lectures, discussions, and informational materials developed based on national guidelines and literature. Data will be analyzed using SPSS, with appropriate statistical tests applied to compare pre- and post-intervention changes between groups.

The aim of this study is to assess the effectiveness of a structured educational intervention grounded in the Theory of Planned Behavior(TPB) in increasing the intention to utilize Breast Cancer Screening service among adult women of Pokhara Metropolitan, Kaski, Nepal. Materials and Resources: Educational content developed by the researcher specifically for this study based on TPB constructs by thorough literature review, research supervisor and public health experts consultation. The leaflet, flipchart and videos publicly available developed by National Health Education and Information Centre(NHEICC), Ministry of Health and Population, Nepal will be used for the educational intervention. Intervention toolkit: Printed handsout and presentation slides created for this study to support 2 hours educational session. Approval: The approval to use the leaflets, flipcharts, videos as the health education material and to distribute to the study participants was taken from NHEICC. Procedures and Timeline: Pre Test (week-1): One week before educational intervention, a structured questionnaire assess baseline knowledge and TPB constructs in the intervention group. Education session (week-2) : Two hours, face to face group session to be delivered in Nepali language. Content: Mini lecture on Breast Cancer, it,s current situation in national and global, methods of Breast Cancer screening, warning signs and risk factors of breast cancer, • Importance, target group, frequency, benefits, possible risk, accessibility, availability, cost of mammography, Myths and facts about mammography, group discussion and distribution of study's educational materials. Facilitators: Principal investigator Post test(week 6): 4 weeks after the educational intervention, the same questionnaire will be readministered to measure changes in knowledge and TPB cinstructs. Total intervention period: 4 weeks Delivery details: Mode: In person, group setting Language: Nepali Location: Ward 30 of Pokhara Metropolitan City Adaptations: Flexible scheduling and reminder calls will be used to maximize study participants attandance. Comparator / Control Treatment: Adult women from ward 27 of Pokhara Metropolitan City Assessments: Pre-test: Administerd at the same time as the intervention group pre-test Post-test: Administered 5 weeks after baseline assessment. No intervention: Control participants wont receive any educational intervention during the 5 weeks period. Ethical fairness: After completion of the post-test, the same educational content will be provided to all control groups. Strategies used to monitor adherence to the intervention: An attendance checklist will be used to record participants’ presence during the health education session using unique identification codes. Attendance will be taken at the beginning and end of the session to ensure full participation. Only those who attended the complete session will be considered adherent to the intervention. Also, unique identification codes to be used to match pre-test and post-test responses, ensuring that only participants who received the intervention be included in the final analysis.

Inclusion criteria

Women between the age 40-75 years, who will be present on the day of study, who will give informed consent and those willing to participate in the study will be included.

Exclusion criteria

Adult women suffering from breast cancer, participated in any educational session training related to breast cancer, FCHVs and women from Health field will be excluded from the study.
