Brief summary

Reduced neck strength is a risk for concussion in some sports. This study will evaluate the effectiveness of a cervical strengthening and stability program against a control group. We hypothesise that the intervention program will significantly increase strength compared to the control group.

Interventions

The intervention group with receive a neck strengthening and stability program. Cervical isometric strengthening will involve resisted static contraction of flexion, extension, and lateral flexion (left and right). Resistance will be provided by participants own hand(s). Each direction will involve one contraction of 15 seconds duration and there will be 30 seconds rest between efforts. The intervention group with receive a neck strengthening and stability program. Cervical isometric strengthening will involve resisted static contraction of flexion, extension, and lateral flexion (left and right). Resistance will be provided by participants own hand(s). Each direction will involve one contraction of 15 seconds duration and there will be 30 seconds rest between efforts. Cervical reflex stability will involve an eccentric sit up without head support (flexors), catch onto hands from kneeling position (extensors), and rolling into side lying positions (lateral flexion – left and right). Each stability exercise will be 30 seconds in duration. The program will completed in a group supervised session once per week and two independent sessions twice per week. The supervised sessions will be facilitated by a physiotherapist, exercise physiologist, or exercise scientist. The program will run for 12 weeks. Adherence will be measured using self report (exercise diary).

Inclusion criteria

Junior AFL footballers aged 14-18 years Play representative competition

Exclusion criteria

- Previous concussion or injury to the neck causing inability to train/play or participate in work /studies: previous injury is likely to require individualised intervention for effective rehabilitation (rather than group preventative exercise) - Recent neck pain (within previous three months) or current neck pain (greater than 2/10 on numerical pain rating scale at rest or during warmup prior to strength testing): previous injury is likely to require individualised intervention for effective rehabilitation (rather than group preventative exercise) - Diagnosed neurological or rheumatological condition - Contextual factors affecting ability to participate (e.g. unavailability)
