Brief summary
This study will test whether a supervised high-intensity strength training program is safe, practical, and acceptable for people living with mild-to-moderate Parkinson’s disease. Participants will complete a structured gym-based strength training program over approximately 8 weeks, with testing before and after the program. The study will monitor safety (including any adverse events), recruitment and attendance, and changes in physical function, mobility, balance, strength, brain health and quality of life. Some participants and trainers will also be interviewed about their experience. We hypothesise that high-intensity strength training will be safe and feasible to deliver in this population, and may lead to improvements in strength and physical function.
Interventions
Twice-weekly high-intensity progressive resistance training sessions are delivered over 8 weeks in a gym setting for community-dwelling people with Parkinson’s disease. Each session will be 1hr in length and is provided on a one-to-one basis, with a participant paired with a Masters-level student trainer (physiotherapy, osteopathy, or exercise physiology) who has completed a structured train-the-trainer workshop and competency process. All sessions occur under direct on-site supervision from qualified allied health clinical staff. Training intensity will be monitored using rating of perceived exertion (RPE), with working sets typically targeting RPE 7–8/10, alongside structured programming (generally 2–3 sets of 6–8 repetitions with 2–3 minutes rest). Load prescription is informed by 3-repetition maximum (3RM) testing for key exercises (e.g., leg press, chest press) at baseline and post-intervention, with loads progressed conservatively based on technique, symptoms, and safety. Attendance and adherence will be recorded at every session using structured trainer logs documenting session completion, loads, RPE, and any symptoms. Prior to each session, participants are screened for fatigue, soreness, and symptom changes, and any adverse events are documented and managed according to study procedures. Exercises target major muscle groups using machine-based and/or free-weight movements tailored to individual capacity and may include leg press, chest press, knee extension/flexion, rowing or pull-down variations, hip hinge movements, and hip abduction. Sessions will include a light aerobic and mobility warm-up, a progressive resistance training block, and a cool-down involving stretching or breathing exercises.

Inclusion criteria
A confirmed diagnosis of Parkinson’s disease, present with mild-to-moderate motor disability (Hoehn and Yahr stage I–III), are aged 18–80 years, and are able to understand English and follow instructions to safely complete the training, testing and study
Exclusion criteria
Individuals will be excluded if they are unable to safely participate in high-intensity strength training due to medical contraindications, or are unable to complete the required assessments and study procedures. Participants will also be excluded where significant comorbidities or impairments would limit safe exercise participation or the valid completion of testing. Examples include orthostatic hypotension; clinically relevant cardiovascular conditions; respiratory/lung conditions; musculoskeletal conditions (e.g., back pain or arthritis) or current injury; uncontrolled diabetes; mood or psychiatric disorders; chronic fatigue syndrome; cognitive dysfunction; unmanaged medical or surgical conditions; severe auditory or visual impairment; cancer; obesity; bone disease; or if currently receiving deep brain stimulation or using Parkinson’s medication infusion/pump systems.
