Brief summary
Reducing dementia risk score through an 8-week structured, personalised & comprehensive multidomain lifestyle program. This program includes physical exercise, MIND diet, cognitive engagement, personalised coaching, health monitoring and dementia specific risk education. Primary objective is to reduce dementia risk score as measured by the cogdrisk assessment tool from baseline to immediately following the 8-week program. The study will also assess the impact that the program will have on dementia knowledge, sleep, mood, diet, physical exercise, cognition, cognitive engagement, loneliness, health related quality of life, activities of daily living and personalised goals. Feasibility of a dementia prevention program at St Vincents Hospital will be monitored using the RE-AIM (reach, effectiveness, adoption, implementation and maintenance) framework.
Interventions
Reducing dementia risk score through an 8-week structured, personalised & comprehensive multidomain lifestyle program. Participants will participate in all components of the program, but it will be tailored to meet their goals and risk factors as assessed with the cogdrisk dementia risk tool.. Programs include twice weekly physiotherapy group exercise classes (30-40minutes of aerobic, strength, stretching and balance training. Gym program sheet for each participant. Exercise will include free wights, cables, theraband, body weight and 'cardio' including treadmill, bike etc.), group education sessions held twice weekly (15-20minutes following physiotherapy class covering dementia knowledge and dementia risk factors), home based exercise programs (aiming for 4 sessions to reach the Australian government recommended target of 150-300minutes of moderate intensity exercise and strength training x2 weekly, exercise programs personalised and will be provided to participants by the study physiotherapist), MIND diet information given at the start of the program and during a education sessions in the first week of the program following physiotherapy exercise class (information will be general from established web pages including dementia Australia and universities. Information given will be given within the context of the NHMRC dietary guidelines that provide evidence-based recommendations for healthy eating to promote health and reduce the risk of chronic diseases among Australians) MIND diet information will be given by the study coordinator, and participants can monitor adherence in their lifestyle diary. MIND diet food is rich in leafy green vegetables, legumes, berries, wholegrains, fish, nuts, and olive oil and low in red and processed meats and discretionary foods (high in saturated fat, added sugar and added salt). Cognitive engagement at home (3-4 sessions of 30minutes per week for the duration of the 8 week program and can include Brain Track and strategies on Dementia Australia prevention page including reading, puzzles, socialisation, painting, cooking and learning a language), Cognitive engagement logged in the participant lifestyle diary. Personalised coaching (weekly for 15 minutes across the 8-week program via telephone to encourage adherence, discuss barriers and develop strategies to assist in participation). The lifestyle diary will be discussed during the personalised coaching and notes documented in a call log. Health monitoring (Blood pressure, heart rate, respiratory rate and oxygen levels assessed at each physiotherapy class). Programs are designed to reflect participants individual dementia risk factors & personalised goals. While the physiotherapy exercise classes are delivered in a group, the program will be personalised. General practitioners who refer participants to the program will be provided with an update following assessment and conclusion of the program for ongoing health monitoring and management of any dementia risk factors.

Inclusion criteria
Adults aged 18 years and over Able to provide informed consent Referred from their general practitioner with medical and safety clearance to participate. Subjective cognitive decline and/or functional cognitive decline and/or asymptomatic with health concern and/or family history of dementia and/or known dementia risk factor/s. • Willingness to engage in an 8-week program with staged clinical follow up across a period of up to two years at St Vincents Hospital Sydney (SVHS) including collection of health data via my medic watch.
Exclusion criteria
Medically unfit to participate in programs components, as assessed by general practitioner. Unable to mobilise independently (i.e. requires assistance to stand and walk) Likely or confirmed diagnosis of Alzheimer’s or dementia as per referring clinician. Significant comorbidities that would prevent safe participation as assessed by the general practitioner or programme clinician.
