Brief summary
This randomized controlled study will be carried out at Dr. Ziauddin Hospital and Al-Rayaz Hospital, Karachi. Participants will be randomly placed into one of two groups: one group will receive Proprioceptive Neuromuscular Facilitation pattern exercises and the other will receive conventional strengthening exercises, with both groups also receiving heat therapy. All participants will attend 18 treatment sessions for six weeks. It is hypothesized that patients receiving Proprioceptive Neuromuscular Facilitation pattern exercises will improve more in pain relief, knee movement, balance, and daily activities than those doing conventional strength training exercises.
Interventions
Experimental Intervention: Diagonal Proprioceptive Neuromuscular Facilitation (PNF) Patterns Exercises + Heat Therapy. - Diagonal Proprioceptive Neuromuscular Facilitation Patterns Exercises consists of Diagonal 1 and Diagonal 2 Lower Limb Patterns of PNF exercises. -For D1 lower limb pattern exercise, the position of the patient will be in supine lying with hip extension-abduction-internal rotation, knee extension and ankle plantar flexion. The therapist will first demonstrate the movement of D1 flexion pattern and then ask the patient to perform it against the resistance. And then the D1 extension pattern exercise will be performed. -For D2 lower limb pattern exercise, the position of the patient will be in side-lying with hip extension-adduction-internal rotation. The therapist will first demonstrate the movement of D2 flexion pattern and then ask the patient to perform it against the resistance. And then the D2 extension pattern exercise will be performed. - Each exercise will be performed as 10 repetitions of 3 sets. A 20 seconds rest will be given in between the two sets. -The intensity of exercise will be associated with the perception of effort between 11 and 13 on Borg scale of rate of perceived exertion. - A total of 18 sessions will be given for 6 weeks on alternative days i.e. 3 sessions per week. -The total duration of each session will be of 35 minutes. - Heat therapy will be given for 10 minutes by the help of electric heating pad. - Intervention will be given by an experienced physiotherapists having more than 5 years of clinical experience. -A diary will be kept in the study setting where the patient attendance will be noted and research supervisor will monitor the adherence to the intervention.

Inclusion criteria
- Both male and females with an age range between 40- 60 years. - Patients diagnosed with unilateral or bilateral knee osteoarthritis. - Grade-II or grade-III on kellgren and lawrence classification. - Duration of knee osteoarthritis more than 3 months (chronic). - Patients having pain intensity more than 3.4 cm on VAS as minimum cut off value.
Exclusion criteria
- Patients with any neurological disorder. - Inflammatory rheumatic disease such as rheumatoid arthritis and gout. - Cancer or any other malignancy such as bone tumor. - Septic knee arthritis. - Knee deformities such as contracture, genu valgum and genu varum. - History of ligamentous injury and meniscal tear. - Any history of lower limb arthroplasty or any other knee surgery.
