Brief summary
Preventive care is a critical part of the Australian and NSW health system. A key component of preventive care is addressing modifiable risks such as smoking/vaping, poor nutrition, alcohol misuse, physical inactivity, and weight (SNAP-W). We will test if a digital tool (+ usual care) is a more effective way than usual care alone for people on a orthopaedic outpatient waiting list to receive preventive care over a 12 week period. We will use three primary outcomes: i) receipt of assessment for SNAP-W risks, ii) receipt of advice for relevant SNAP-W risks, and iii) receipt of a referral for at least one support service for relevant SNAP-W risks.
Interventions
Participants will receive usual care while on the orthopaedic outpatient waiting list, plus the digital tool over a 12 week period. Participants will be invited to participate in the study via an SMS containing a link to the Participant Information Sheet and Consent Form, and then the digital tool for those randomised into the intervention group. The digital tool will be a user-friendly REDCap-based platform that will be accessible to participants on any internet-connected device (smartphone, laptop, etc.) The content of the digital tool will be based on the recommended AAH approach (Ask/Advise/Help) to preventive care for SNAP-W (smoking, nutrition, alcohol, physical activity, and weight) risks, as outlined below. The initial assessment will be spread across three weeks (week 1- nutrition and weight, week 2- physical activity, week 3- alcohol and smoking/vaping) and should take the participant 5-10mins to complete each week. The second assessment at week 8 will include all SNAP-W assessments and should take participants 10-20mins to complete. Engagement with the digital tool (including assessment completion, review of advice and help sections) will be monitored using digital tool analytics. Assess Using the digital tool, participants will self-assess their smoking/vaping, nutrition, alcohol, physical activity and weight risk. The digital tool will incorporate valid and reliable assessments where available. The assessments will align with those currently used in other clinical settings in the local health district and Australian guidelines. The assessments for all participants are: o Smoking/Vaping: smoking/vaping status and frequency, and nicotine dependence assessment o Nutrition: Cappuccio Diet Questionnaire o Alcohol: AUDIT-C o Physical Activity: questions from International Physical Activity Questionnaire (IPAQ) o Weight: weight and height (measured by the participant), to enable calculation of Body Mass Index and weight status Advise Following the Assess step, all participants will be provided with guideline-based information on health risks (smoking/vaping, nutrition, alcohol, physical activity and weight) tailored to the outcomes of their assessment. Advice will be provided to participants for their relevant SNAP-W risks, i.e. those risks where the participant is not meeting guidelines. Intervention group participants will receive advice within the digital tool, while both groups should receive AAH care for their SNAP-W risks from any health professional they see during the study period. Help All participants will be provided with information about free health coaching services and clinical health services to which they can self-refer for their relevant SNAP-W risks, i.e. those risks where the participant is not meeting guidelines (see below). These services align with current referral pathways of various clinical settings in Hunter New England Local Health District and are services recommended by the NSW Ministry of Health. Support services will include: - Quitline and Aboriginal Quitline: for smokers/vapers and recent quitters - Get Healthy Service: for weight, physical activity, nutrition and alcohol - Drug & Alcohol Services: for alcohol (high risk consumption) - Aboriginal Medical Services: for Aboriginal and Torres Strait Islander people for smoking/vaping cessation, alcohol counselling, and dietitians, as available at individual Aboriginal Medical Services.

Inclusion criteria
- Any person aged 18 years and over on the orthopaedic consultation waiting list at one of the two participating outpatient orthopaedic services - Have a mobile phone number on record (to be able to receive the digital tool) - Sufficient level of English language proficiency to access and use the digital support tool and complete follow-up surveys
Exclusion criteria
- No registered mobile phone number - Currently pregnant
