Brief summary
Chronic Kidney Disease (CKD) affects 1 in 10 adults in Australia and, if unmanaged, can lead to heart disease and kidney failure. This clinical trial aims to compare the effectiveness of a structured, group-based multidisciplinary education program for people with CKD Stage 3a and 3b against usual care. The program includes five group sessions focused on managing blood pressure, diabetes, weight, smoking, diet, exercise, medications, and personal health planning. Key outcomes include changes in kidney function, lifestyle behaviours, quality of life, and cost-effectiveness, with additional evaluation of patient and carer experiences and feasibility for routine implementation. The study also seeks to determine the cost utility of the revised model from the perspective of the Australian healthcare system.
Interventions
The intervention consists of five group-based, education and skill development sessions held once per week over a 7 week period (depending on participant availability). These sessions will be led by a renal trained CKD Clinical Nurse Specialist (CNS2) with nursing and allied health support. Each session is approximately 2 hours in duration with time for non-supervised peer support. Sessions begin with 45-60 minutes of education and skills education followed by 30minutes of community-based activity (e.g., walking, yoga, strength exercises, dancing). Topics include: - Understanding CKD & Patient Experience (CNS2, RN & Dialysis Patients) - Maintaining Kidney Health (CNS2, RN) - Eating Well for Kidney Health (CNS2 & Dietitian) - Medication Management for Kidney Health (CNS2 & Pharmacist) - Living Your Best Life – Developing a Personal Kidney Health Plan (CNS2, Social Work & Physiotherapist)

Exclusion criteria
Exclusion criteria are those who cannot attend the service or cannot speak English. Safety to participate will be discussed by a member of the research team or the patient with the treating doctor. Contraindications may include short life expectancy < 6 months or unstable cardiac function precluding unsupervised exercise .
