Compare health outcomes of people with a severe mental illness and at least one co-morbid chronic physical health problem who attend the Physical Health Clinic at Fremantle Hospital with a similar group of people who are engaged with a general practitioner in the community.

Interventions

Exploratory comparative 12 month study of the physical and mental health outcomes of patients being case managed by Fremantle Mental Health Services who attend a Physical Health Clinic at the service and a similar group of patients who see a general practitioner in the community setting using a within subject and between group design.

18 Years to 65 Years

Healthy volunteers

Inclusion criteria

Severe mental Illness with at least one co-morbid chronic physical health problem and speak and understand English and being case managed by Fremantle Mental Health Services

Exclusion criteria

under 18 years and over 65 years and do not speak English clinical recommendation of the treating psychiatrist Well engaged with a general practitioner
